An annual registration fee of $25.00 per dancer or $40.00 per family is due at the time of registration and is nonrefundable.

Dance tuition is charged automatically to your Credit Card (Visa/Mastercard/Discover) or your ATM card (with Visa symbol) between the 20 th of the prior month and the 5 th of the current month, or Automatic Bill Pay may be set up through your bank. Bill Pay accounts must be received by the studio no later than the 5 th of the month. Tuition paid directly in the studio by check, ATM or credit card, is subject to a 10% service charge. A late fee of $10 will be applied to any tuition payments received after the 5 th of the month. Accounts that remain unpaid will be charged 2% on the balance due monthly until current. There will be a $20.00 service charge for all checks returned from the bank. Tuition is nonrefundable.

Full tuition is due regardless of holidays or missed classes. Similarly, Foothills Dance and Performing Arts Studio does not increase the monthly fee when there are five classes in a month. Missed classes may be made up in a similar class with a similar or lower skill level. No refunds or prorated credits will be given for missed classes. Foothills Dance and Performing Arts Studio reserves the right to close for holidays and scheduled breaks. Please refer to the studio calendar for scheduled breaks and holidays.

We do not allow parents or siblings in the dance room during class time.

Participants are required to adhere to Foothills Dance and Performing Arts Studio dress codes. To maintain the quality of our floors, dance shoes should be carried in and out of the studio and not worn outside.

Foothills Dance and Performing Arts Studio makes every effort to provide a safe atmosphere and is committed to teaching level appropriate and injury free dance movements during class. Dance is a physical activity and injuries may occur. I acknowledge that Foothills Dance and Performing Arts Studio instructors and staff do not provide medical insurance coverage for my child and will not be held responsible for injuries occurring before, during or after class on studio premises or traveling to and from, and participating in dance recitals, competitions, or other performances.

I understand and agree that Foothills Dance and Performing Arts Studio does not and cannot assume any financial responsibility for any medical assistance or care my child may require. I agree to hold Foothills Dance and Performing Arts Studio, its owners, instructors, and staff members harmless for any injury or damage incurred on or off site.

Parent or legal guardian shall be solely responsible at all times for any and all acts of the participant or siblings which may result in injury to the participant or other persons or damage to any property.

Parent or legal guardian shall be financially responsible for tuition and studio related fees for participant.

Foothills Dance and Performing Arts Studio instructors and staff are not responsible nor will they be liable for loss or damage to personal property brought into the studio by participants.

Foothills Dance and Performing Arts Studio reserves the right to refuse service or discontinue service to students for non-payment of tuition or other fees, and any inappropriate or disruptive behavior by the dancer, siblings, or parents that is deemed detrimental to the studio, its staff, or instructors.

Foothills Dance and Performing Arts Studio may use photographic and/or video images of its students for marketing, publicity and advertising purposes. Personally identifiable information will not be given out regarding the images. Foothills Dance and Performing Arts Studio is authorized to use these images without pay or compensation. All representations of the photographic images, copies, etc. constitute Foothills Dance and Performing Arts Studio’s property, solely and completely.

If you decide to discontinue class, please notify the office in writing at least two weeks prior to the beginning of the next month. Verbal communication with an instructor is not sufficient. Unless written notification has been received by the first day of the month, you will be charged normal tuition fees for that month.
